THE RELATIONSHIP BETWEEN PREOPERATIVE NUTRITIONAL STATUS AND POSTOPERATIVE COMPLICATIONS AND LENGTH OF HOSPITAL STAY IN PATIENTS WITH PANCREATIC CANCER AT THE VIET NAM NATIONAL CANCER HOSPITAL 2023-2024

Mục tiêu: Nghiên cứu thực hiện nhằm tìm hiểu mối liên quan giữa tình trạng dinh dưỡng trước phẫu thuật với biến chứng sau phẫu thuật và thời gian nằm viện của người bệnh mắc ung thư tụy.
Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang tiến hành trên 41 người bệnh được chẩn đoán ung thư tụy có chỉ định phẫu thuật tại Bệnh viện K từ tháng 5/2023 đến tháng 3/2024. Người bệnh được đo chiều cao, cân trọng lượng cơ thể và phân loại tình trạng dinh dưỡng theo BMI, đánh giá nguy cơ dinh dưỡng theo PG-SGA, chỉ số hóa sinh Albumin huyết thanh, biến chứng sau phẫu thuật và thời gian nằm viện từ hồ sơ bệnh án.
Kết quả: Người bệnh có nguy cơ suy dinh dưỡng theo PG-SGA rất cao lên tới 82,9%, suy dinh dưỡng theo BMI là 24,4% và theo Albumin huyết thanh là 26,8%. Người bệnh có biến chứng sau phẫu thuật, tỷ lệ nguy cơ suy dinh dưỡng theo PG-SGA trước phẫu thuật là 100% trong khi ở người bệnh không có biến chứng tỷ lệ này là 80%, tuy nhiên kết quả chưa có ý nghĩa thống kê với p>0,05. Người bệnh có nguy cơ suy dinh dưỡng theo PG-SGA có thời gian nằm viện dài hơn (13,9±8,1 ngày) so với không có nguy cơ suy dinh dưỡng (9,4±2,3 ngày), kết quả có ý nghĩa thống kê với p=0,01.
Kết luận: Cần đánh giá tình trạng dinh dưỡng toàn diện cho người bệnh nhập viện và can thiệp dinh dưỡng tích cực trước phẫu thuật để rút ngắn thời gian nằm viện và giảm thiểu rủi ro biến chứng sau phẫu thuật ở người bệnh ung thư tụy.
Abstract
Objective: The study aims to explore the relationship between preoperative nutritional status and postoperative complications, as well as the length of hospital stay, in patients with pancreatic cancer.
Patients & Methods: A cross-sectional descriptive study was conducted on 41 patients diagnosed with pancreatic cancer who were indicated for surgery at the Viet Nam National Cancer Hospital from May 2023 to March 2024. Patients were measured for height, body weight, and classified by nutritional status according to BMI. Nutritional risk was assessed using the PG-SGA and biochemical indices such as serum albumin were recorded. Postoperative complications and the length of hospital stay were obtained from medical records.
Results: The study found that patients had a very high risk of malnutrition according to the PG-SGA, reaching up to 82.9%. Malnutrition rates were 24.4% according to BMI and 26.8% based on human serum Albumin. Among patients with postoperative complications, the preoperative malnutrition risk according to the PG-SGA was 100%, compared to 80% in patients without complications; however, this result was not statistically significant (p>0.05). Patients at risk of malnutrition according to the PG-SGA had a longer hospital stay (13.9±8.1 days) compared to those not at risk (9.4±2.3 days), with the result being statistically significant (p=0.01).
Conclusion: A comprehensive nutritional assessment is essential for hospitalized patients, and proactive nutritional interventions before surgery are necessary to shorten hospital stays and minimize the risk of postoperative complications in patients with pancreatic cancer.
