Đặt vấn đề: Ung thư dạ dày là một trong những loại ung thư phổ biến trên thế giới và ở Việt Nam. Vai trò của hóa trị hỗ trợ sau cắt dạ dày tiêu chuẩn đem lại lợi ích về sống còn cho bệnh nhân carcinoma tuyến dạ dày giai đoạn II-III, trong đó TS-1 là một dẫn xuất của fluorouracil đang được sử dụng ngày càng rộng rãi ở Việt Nam nói riêng và khu vực châu Á nói chung.
Đối tượng và phương pháp nghiên cứu: nghiên cứu mô tả hồi cứu kết hợp tiến cứu, cập nhật kết quả lần cuối trên 45 bệnh nhân carcinoma tuyến dạ dày giai đoạn II-III được điều trị với phác đồ TS-1/SOX hỗ trợ tại bệnh viện Ung Bướu TPHCM từ tháng 1/2020 đến tháng 12/2022. Mục tiêu đánh giá: thời gian sống còn không tái phát, thời gian sống còn toàn bộ, tính dung nạp và tác dụng phụ của phác đồ TS-1/SOX hỗ trợ.
Kết quả: Kết thúc theo dõi tại thời điểm 63 tháng, thời gian sống còn không bệnh tái phát (DFS) trung bình là 40,2 tháng (95%CI 33,0-47,3), thời gian sống còn toàn bộ (OS) trung bình là 50 tháng (95%CI 44,3-55,6). Sự khác biệt về DFS và OS giữa 2 nhóm bệnh nhân dùng TS-1 đơn trị và SOX là không có ý nghĩa thống kê. Trung vị sống còn toàn bộ là 57 tháng (95%CI 41,8-72,2). Độc tính đã được báo cáo tại thời điểm 37 tháng theo dõi, với 8,9% bệnh nhân có tác dụng phụ độ 3, không có tác dụng phụ độ 4-5.
Kết luận: Phác đồ hóa trị hỗ trợ có TS-1 trong ung thư dạ dày giai đoạn II-III có DFS và OS trung bình là 45,9 tháng và 47,5 tháng, độ an toàn với độc tính chấp nhận được và khả năng dung nạp cao. Do cỡ mẫu nhỏ và thời gian theo dõi chưa đủ 3 năm ở một số ca nên hiệu quả của phác đồ cần được khảo sát thêm.
Abstract
Background: Gastric cancer is one of the most common malignancies worldwide and in Vietnam. Adjuvant chemotherapy after standard gastrectomy is proved to provide survival benefit for gastric carcinoma stage II-III. TS-1, which is a derivate of fluorouracil, has been commonly used in Vietnam and Asian nations in this setting.
Methods: This is a retroperspectiveand prospective study,updated from the last results on 45 gastric carcinomapatients stage II-III were treated with adjuvant TS-1/SOX regimen at Ho Chi Minh city Oncology Hospital from January 2020 to December 2022.Objectives: relapse-free survival, overall survival and side effectsof TS-1/SOX regimens in adjuvant therapy for stage II-III gastric cancer in Ho Chi Minh city Oncology Hospital.
Results: The mean disease-free survival (DFS) and overall survival (OS) was 40,2 months (95%CI 33,0-47,3) and 50 months (95%CI 44,3-55,6), respectively. There was no significant difference of DFS and OS between TS-1 and SOX groups (p=0,59 and 0,56). Median OS was 57 months (95%CI 41,8-72,2). Adverse effects and tolerability were previously reported: grade 3 adverse effects were recognized in 8,9% patients and no patients had grade 4-5.
Conclusion: The mean DFS and OS of adjuvant chemotherapy with TS-1-based regimens in stage II-III gastric cancer is 40,2 months and 50 months. The analysis, on the other hand, indicates these regimens were safe and well-tolerated with acceptable adverse effects. Due to small sample size and short time folow-up, the efficacy of the regimens is needed to be further evaluated.
