THE ROLE OF F18-FDG PET/CT IN EVALUATING LYMPH NODE METASTASIS IN NON-SMALL CELL LUNG CANCER PATIENTS INDICATED FOR CURATIVE SURGERY

Tóm tắt

Mục tiêu:  Độ nhạy và độ đặc hiệu của F18 – FDG PET/CT trong đánh giá hạch di căn trên bệnh nhân ung thư phổi không tế bào nhỏ có chỉ định phẫu thuật triệt căn.

Đối tượng nghiên cứu: Bệnh nhân ung thư phổi không tế bào nhỏ có chỉ định phẫu thuật triệt căn tại bệnh viện Ung bướu TP.HCM, được chụp F18 – FDG PET/CT để đánh giá giai đoạn trước phẫu thuật và có kết quả giải phẫu bệnh của hạch trung thất sau phẫu thuật từ tháng 01/2023 đến tháng 12/2024.

Thiết kế nghiên cứu: Hồi cứu.

Kết quả: 43 bệnh nhân thỏa tiêu chuẩn chọn mẫu, với 120 hạch được khảo sát. PET/CT có độ nhạy 96.7% và độ đặc hiệu 65.6%, giá trị chẩn đoán dương 48.3%, giá trị chẩn đoán âm 98.3%. Các phương pháp đánh giá hạch trên PET/CT bao gồm SULmax của hạch, kích thước trục ngắn của hạch và tỷ số SULmax của hạch/bướu đều cho thấy có khả năng chẩn đoán hạch di căn. Ngưỡng SULmax tối ưu mà chúng tôi xác định là 2.61 với độ nhạy 93.3% và độ đặc hiệu 68.9%.

Kết luận: F18-FDG PET/CT là một công cụ hữu ích trong việc đánh giá hạch di căn ở bệnh nhân ung thư phổi không tế bào nhỏ với độ nhạy cao (96,7%) và giá trị tiên đoán âm rất cao (98,3%). Tỷ lệ dương tính giả cao chỉ ra rằng cần thực hiện thêm chẩn đoán mô bệnh học những trường hợp hạch dương tính trên PET/CT. SULmax của hạch trung thất được xác định là chỉ số có giá trị tiên đoán cao nhất, vượt trội hơn so với kích thước trục ngắn của hạch và tỷ số SULmax hạch/bướu.

Abstract

Objective: To evaluate the sensitivity and specificity of F18-FDG PET/CT in assessing lymph node metastasis in patients with non-small cell lung cancer (NSCLC) indicated for curative surgery.

Subjects: NSCLC patients scheduled for curative surgery at Ho Chi Minh City Oncology Hospital, who underwent F18-FDG PET/CT for preoperative staging and had postoperative histopathological results of mediastinal lymph nodes, from January 2023 to December 2024.

Study Design: Retrospective study.

Results: A total of 43 patients met the inclusion criteria, with 120 lymph nodes examined. PET/CT demonstrated a sensitivity of 96.7%, specificity of 65.6%, positive predictive value of 48.3%, and negative predictive value of 98.3%. Lymph node assessment methods on PET/CT, including lymph node SULmax, short-axis diameter, and the lymph node-to-tumor SULmax ratio, all showed diagnostic potential for metastatic lymph nodes. The optimal SULmax threshold identified was 2.61, yielding a sensitivity of 93.3% and specificity of 68.9%.

Conclusion: F18-FDG PET/CT is a valuable tool for evaluating lymph node metastasis in NSCLC patients, exhibiting high sensitivity (96.7%) and excellent negative predictive value (98.3%). The high false-positive rate indicates the necessity for histopathological confirmation of PET/CT-positive lymph nodes. Among the evaluated parameters, mediastinal lymph node SULmax was identified as the most predictive index, outperforming both the short-axis diameter and the lymph node-to-tumor SULmax ratio.
