EFFECTIVENESS OF ARC-SPLITTING TECHNIQUE IN STEREOTACTIC RADIOSURGERY FOR MULTIPLE BRAIN METASTASES

Mục tiêu: Nghiên cứu này nhằm đánh giá hiệu quả của kỹ thuật tách cung so với kế hoạch 5 cung truyền thống trong xạ phẫu lập thể (SRS) điều trị di căn não đa ổ. Trọng tâm phân tích bao gồm giảm hiện tượng cầu liều, bảo vệ mô não lành và cải thiện tỷ lệ thành công của đảm bảo chất lượng (QA) trên hệ thống TrueBeam không hỗ trợ HyperArc.
Phương pháp: Hồi cứu 22 bệnh nhân (52 ổ di căn, 2–4 ổ mỗi ca) tại Bệnh viện Ung Bướu TP. HCM. Mỗi bệnh nhân được lập hai kế hoạch: (1) kế hoạch 5 cung VMAT (2 toàn cung đồng phẳng và 3 bán cung không đồng phẳng), và (2) kế hoạch tách cung VMAT (tối đa 10 cung, có đồng phẳng và không đồng phẳng, mỗi cung bao 1–2 ổ gần nhau). So sánh các chỉ số liều (D2%, D98%, Dmean, CI, CI50, HI, GI), liều não lành (V10%, V25%, V12 Gy), số MU và tỷ lệ QA (2%2mm, 1.5%1.5mm).
Kết quả: Cả hai phương pháp đều đạt độ bao phủ và độ phù hợp PTV tương đương (D2, D98, Dmean, CI, HI, GI), đáp ứng các tiêu chuẩn RTOG. Kỹ thuật tách cung giảm đáng kể liều não lành (V10% giảm 11,95%; V25% giảm 13,49%; V12Gy giảm 6,65%) và hạn chế cầu liều, nhưng làm tăng số MU lên 1,72 lần. Tỷ lệ QA cải thiện rõ rệt với tách cung (31,8% → 100% ở mức 2%2mm; 9% → 73% ở mức 1,5%1,5mm).
Kết luận: VMAT tách cung là lựa chọn thực tế và hiệu quả trong SRS cho bệnh nhân di căn não đa ổ trên hệ thống không hỗ trợ HyperArc. Kỹ thuật này giúp bảo vệ não lành tốt hơn và tăng tính ổn định của QA mà không ảnh hưởng đến bao phủ khối u, dù thời gian phát tia dài hơn. Đây là giải pháp đặc biệt hữu ích cho các trung tâm vẫn sử dụng nền tảng TrueBeam/Eclipse phiên bản cũ.
Abstract
Objective: This study aimed to evaluate the effectiveness of the arc-splitting technique compared with the conventional 5-arc plan in stereotactic radiosurgery (SRS) for multiple brain metastases. The focus was on reducing dose bridging, sparing healthy brain tissue, and improving plan quality assurance (QA) pass rates on TrueBeam systems without HyperArc support.
Methods: A retrospective cohort of 22 patients (52 lesions, 2–4 per case) treated at Ho Chi Minh City Oncology Hospital was analyzed. Each patient underwent two plans: (1) a 5-arc VMAT plan (two coplanar full arcs and three non-coplanar half arcs) and (2) an arc-splitting VMAT plan (up to 10 arcs, each covering 1–2 adjacent lesions, coplanar full arcs and non-coplanar half arcs). Dosimetric indices (D2%, D98%, Dmean, CI, CI50, HI, GI), healthy brain dose (V10%, V25%, V12 Gy), monitor units (MU), and QA pass rates (2%2mm, 1.5%1.5mm) were compared.
Results: Both techniques provided equivalent target coverage and conformity (D2, D98, Dmean, CI, HI, GI), consistent with RTOG standards. Arc-splitting significantly reduced healthy brain dose (V10% reduced by 11.95%; V25% reduced by 13.49%; V12Gy reduced 6.65%) and minimized dose bridging, at the cost of increased MU (1.72-fold). QA pass rates improved markedly with arc-splitting (31.8%→100% at 2%2mm; 9%→73% at 1.5%1.5mm).
Conclusion: Arc-splitting VMAT is a practical and effective alternative for multiple brain metastases SRS on systems without HyperArc. It provides superior healthy brain sparing and QA robustness without compromising tumor coverage, albeit with longer beam-on time. This approach may be particularly valuable for centers using older TrueBeam/Eclipse platforms
