Tóm tắt
Đặt vấn đề: Huyết khối tĩnh mạch cửa (HKTMC) trong ung thư biểu mô tế bào gan (UTBMTBG) là một yếu tố tiên lượng xấu, làm giảm thời gian sống thêm toàn bộ trung bình xuống dưới 3 tháng nếu không được can thiệp. Trong một số trường hợp, phẫu thuật có thể giúp tăng thời gian sống thêm của bệnh nhân so với các phương pháp khác.
Mục tiêu: Nghiên cứu của chúng tôi báo cáo kết quả phẫu thuật cắt gan điều trị UTBMTBG có huyết khối lan tới nhánh chính của tĩnh mạch cửa tại Bệnh viện K.
Đối tượng và phương pháp nghiên cứu: Đây là nghiên cứu mô tả hồi cứu, gồm 19 bệnh nhân được phẫu thuật cắt gan do UTBMTBG có HKTMC lan tới nhánh phải, nhánh trái hoặc thân chung tĩnh mạch cửa từ tháng 1 năm 2018 đến tháng 12 năm 2024.
Kết qủa: HKTMC Vp3 gặp ở 18 trường hợp (94,7%) và Vp4 ở 1 trường hợp (5,3%), kích thước u lớn nhất trung bình là 70,4 ± 24,7mm. Có 18 bệnh nhân viêm gan B (94,7%) và 1 bệnh nhân xơ gan rượu (5,3%). Phẫu thuật cắt gan phải chiếm 52,6% trường hợp, cắt gan trái 26,3% và cắt gan trái mở rộng 21,1%. Không có bệnh nhân nào biến chứng Clavien-Dindo độ III trở lên. Trung vị thời gian sống thêm toàn bộ là 21,3 ± 3,7 tháng và trung vị thời gian sống thêm không bệnh là 11,2 ± 2,6 tháng.
Kết luận: Phẫu thuật cắt gan có hiệu quả và an toàn đối với bệnh nhân UTBMTBG có huyết khối nhánh chính tĩnh mạch cửa.
Abstract
Background: Portal vein tumor thrombus (PVTT) in hepatocellular carcinoma (HCC) is a poor prognostic factor, reducing the median survival time to less than 3 months in the absence of treatment. In selected cases, surgical resection may offer a survival benefit over other treatment modalities.
Methods: A retrospective descriptive study was conducted at the Vietnam National Cancer Hospital from January 2018 to Decembre 2024 to evaluate the outcomes of hepatic resection for HCC with tumor thrombus involving the major portal vein.
Results: 19 patients underwent hepatectomy for HCC with tumor thrombus in the first branch or trunk of the portal vein. Vp3 PVTT was observed in 18 patients (94.7%), and Vp4 in 1 patient (5.3%). The mean maximum tumor diameter was 70.4 ± 24.7 mm. All patients had chronic liver disease: hepatitis B in 94.7% and alcoholic cirrhosis in 5.3%. Right hepatectomy was performed in 52.6% of cases, left hepatectomy in 26.3%, and extended left hepatectomy in 21.1%. No patient had Clavien-Dindo postoperative complications grade ≥ III. The median overall survival and the median disease-free survival was 21.3 ± 3.7 months and 11.2 ± 2.6 months, respectively.
Conclusions: Hepatic resection is safe and provides benefits for HCC patients with PVTT in the major portal vein.
