THE ORMOND’S DISEASE (RETROPERITONEAL FIBROSIS – RPF): A CASE REPORT AND LITERATURE REVIEW
Xơ hóa sau phúc mạc (bệnh Ormond) là bệnh lý hiếm gặp, đặc trưng bởi một tình trạng xơ hóa lan tỏa và viêm khoang sau phúc mạc, bao quanh động mạch chủ bụng, niệu quản và các cơ quan khác trong ổ bụng. Chẩn đoán RPF thường khó khăn do hình ảnh X-quang và mô bệnh học đa dạng, dễ gây nhiễu chẩn đoán. Chúng tôi báo cáo một trường hợp bệnh nhân nữ 58 tuổi được chẩn đoán RPF với bối cảnh lâm sàng đau bụng, thiếu máu nhẹ. Các xét nghiệm hình ảnh (siêu âm, CT/MRI) cho thấy khối giảm âm/đồng nhất quanh động mạch chủ bụng dưới và động mạch chậu chung. Giải phẫu bệnh cho thấy hình ảnh viêm xơ hóa. Bệnh nhân đáp ứng tốt với liệu pháp Corticoid, MRI sau 8 tháng cho thấy tổn thương gần như thoái triển hoàn toàn RPF là bệnh hiếm gặp, nguyên nhân và sinh bệnh học chưa rõ ràng, thường biểu hiện đau bụng/lưng. CT/MRI là phương pháp chẩn đoán và theo dõi ưu tiên. Mô bệnh học có giá trị quyết định, cần có sự phối hợp với Xquang khi gặp khó khăn ở trường hợp mẫu sinh thiết nhỏ.
Liệu pháp Corticoid là lựa chọn đầu tiên, các thuốc ức chế miễn dịch và tác nhân sinh học đang được đánh giá. Giải phóng niệu quản bị chèn ép được thực hiện khi cần thiết. Nhìn chung, bệnh có tiên lượng tốt
Abstract
Retroperitoneal fibrosis (RPF) or The ORMOND’S disease is a rare inflammatory and fibrous condition encircling the abdominal aorta, ureters, and other abdominal organs. Accurate diagnosis requires understanding its variable imaging and histopathological features. We report a case of a 58-year-old female diagnosed with Ormond’s disease, presenting with abdominal pain and mild anemia. Ultrasound revealed hypoechoic structures around the aorta, while CT/MRI showed a homogenous lesion surrounding the lower abdominal aorta and iliac arteries. Histopathology confirmed inflammatory and fibrous changes. Corticoid therapy was initiated, leading to a good response; MRI after eight months showed no significant change in lesion size or appearance. RPF's etiology and pathogenesis remain unclear, often presenting with abdominal/low back pain. Inflammatory markers may be elevated. Typical CT/MRI findings include a homogenous mass surrounding the aorta and iliac arteries, potentially displacing ureters. On MRI, lesions are T1 hypointense and T2 variable (low in quiescent, high in active stages). PET/CT aids in evaluating inflammatory activity. Microscopic examination reveals fibrous tissue with inflammatory cells. CT/MRI are preferred for diagnosis and follow-up. Approximately half of RPF cases are linked to IgG4-related disease. RPF is often misdiagnosed as malignancy due to variable clinical and radiological presentations, making pathological diagnosis challenging on small biopsies. Corticosteroids are first-line, with immunosuppressants and biological agents also used. Ureteric obstruction may require stenting or ureterolysis. Prognosis is generally good.
