TREATMENT OUTCOME OF NEOADJUVANT THERAPY WITH TCHP REGIMEN IN PATIENTS WITH HER2+ BREAST CANCER AT HANOI ONCOLOGY HOSPITAL

Abstract
Objective: To evaluate the efficacy of neoadjuvant treatment using the TCHP regimen in patients with HER2-positive breast cancer at Hanoi Oncology Hospital.
Materials and Methods: A combined retrospective and prospective cross-sectional descriptive study was conducted on patients with HER2-positive breast cancer treated with the TCHP regimen as neoadjuvant at Hanoi Oncology Hospital from January 2023 to September 2025.
Results: Among 36 enrolled patients, the average age was 48.6 years (range 30–70). There were 20 patients (55.6%) with stage II and 16 patients (44.4%) with stage III disease; 14 patients (38.9%) underwent clip placement before treatment. After 3 cycles, 7 patients (19.4%) achieved clinical complete response; after 6 cycles, this rate doubled to 14 patients (38.9%). The pathological complete response (pCR) rate was achieved in 24 patients (75%). Clinical complete response was a predictive factor for achieving pCR (p < 0.05), while no significant association was found between pCR and age, stage, histological grade, hormone receptor status, or Ki-67. No patients experienced disease progression during treatment. Most adverse events (AEs) were at grade 1–2. The most common grade 3–4 AE was neutropenia (3 patients, 8.3%). No grade 3–4 thrombocytopenia, hepatic, renal, or cardiac toxicities were observed. Two patients (5.6%) experienced grade 1 left ventricular ejection fraction reduction without requiring treatment delay.
Conclusion: The TCHP regimen demonstrates high efficacy with good tolerability and manageable, predictable AEs in patients with HER2+ breast cancer.
