Quality of life assessment based on self-reported outcomes of breast cancer patients after adjuvant radiotherapy at Vinmec Times City International General Hospital
Mục tiêu: Đánh giá chất lượng cuộc sống (CLCS) dựa trên kết quả tự báo cáo (PROMs-Patient Reported Outcome Measures) của các bệnh nhân ung thư vú sau xạ trị bổ trợ tại Bệnh viện Đa khoa Quốc tế Vinmec Times City.
Đối tượng và phương pháp: Nghiên cứu mô tả cắt ngang trên 43 bệnh nhân ung thư vú được xạ trị bổ trợ từ tháng 1/2024 đến tháng 6/2025 bằng thang điểm đánh giá hiệu quả điều trị ung thư – ung thư vú (FACT-B), do nhóm đánh giá hiệu quả điều trị bệnh mạn tính (FACIT – Functional Assessment of Chronic Illness Therapy) phát triển. FACT-B gồm 37 câu đánh giá 5 lĩnh vực: thể chất, xã hội/gia đình, cảm xúc, chức năng và các vấn đề đặc hiệu ung thư vú. Phân tích bằng t-test và tương quan Pearson.
Kết quả: Điểm FACT-B trung bình: 89,8/148; TOI: 61,9/96 → chất lượng sống trung bình khá. Bệnh nhân >50 tuổi có điểm cảm xúc (EWB) thấp hơn nhóm trẻ hơn (p=0.03). Điểm BMI cao liên quan đến giảm điểm thể chất (PWB) (r=-0.30; p=0.03). Xạ trị 25–30 buổi tương quan với chất lượng sống thấp hơn so với nhóm 15–20 buổi (r=-0.29; p=0.03). Phẫu thuật bảo tồn vú giúp duy trì chất lượng sống tốt hơn so với cắt bỏ toàn bộ, đặc biệt ở chỉ số BCS (26.0 vs 23.6) và EWB (12.4 vs 11.3).
Kết luận: Bệnh nhân ung thư vú sau xạ trị tại khoa xạ trị BV ĐKQT Vinmec Times City có chất lượng cuộc sống ở mức trung bình khá. Tuy nhiên, nhóm NB cao tuổi, BMI cao, xạ trị nhiều phân liều và phẫu thuật cắt toàn bộ vú có CLCS thấp hơn nhóm còn lại. Do đó, cần xây dựng các chương trình hỗ trợ tâm lý và kiểm soát triệu chứng phù hợp cho từng nhóm nguy cơ.
Abstract
Objectives: Assessment of Quality of Life (QoL) Based on Patient-Reported Outcome Measures (PROMs) in Breast Cancer Patients After Adjuvant Radiotherapy at Vinmec Times City International General Hospital.
Methods: A cross-sectional study was conducted on 43 female breast cancer patients treated with 4D-DIBH radiotherapy technique from January 2024 to June 2025. The FACT-B questionnaire, covering five domains—physical well-being, social/family well-being, emotional well-being, functional well-being, and breast cancer-specific concerns—was used. Data were analyzed using t-tests and Pearson correlation.
Results: The mean FACT-B total score was 89.8/148 and TOI was 61.9/96, indicating a moderate to good quality of life. Patients over 50 years old had significantly lower emotional well-being scores (EWB) than younger groups (p=0.03). Higher BMI was associated with lower physical well-being (PWB) scores (r = -0.30; p=0.03). A greater number of radiotherapy sessions (25–30) was significantly correlated with reduced quality of life compared to fewer sessions (15–20) (r = -0.29; p = 0.03). Breast-conserving surgery was associated with better QoL than total mastectomy, especially in BCS (26.0 vs 23.6) and EWB (12.4 vs 11.3) scores.
Conclusion: Breast cancer patients after radiotherapy at Vinmec Times City had a moderate to good quality of life. However, older patients, those with higher BMI, those receiving a higher number of radiotherapy fractions, and those undergoing total mastectomy had lower quality of life compared to their respective counterparts. Psychological support and symptom management programs tailored to risk groups are necessary.
