SURVEY ON NURSES’ KNOWLEDGE AND SKILLS IN CARING FOR PATIENTS WITH SUBCUTANEOUS VENOUS ACCESS PORTS AT THE ONCOLOGY HOSPITAL

Đặt vấn đề: Những năm gần đây, buồng tiêm dưới da (BTDD) ngày càng được sử dụng rộng rãi trong điều trị ung thư nhờ ưu điểm an toàn, ít biến chứng và nâng cao chất lượng cuộc sống cho người bệnh (NB). Tuy nhiên, việc chăm sóc BTDD đòi hỏi Điều dưỡng (ĐD) phải có kiến thức và kỹ năng thực hành chuyên nghiệp. Thực tế cho thấy, nếu không được đào tạo bài bản, kiến thức và thực hành của ĐD chỉ đạt khoảng 50%, làm tăng nguy cơ biến chứng cho người bệnh. Tại Bệnh viện Ung Bướu (BVUB), số lượng NB đặt BTDD ngày càng tăng, song công tác chăm sóc vẫn còn nhiều hạn chế. Do đó, việc khảo sát kiến thức và kỹ năng chăm sóc NB có đặt BTDD của ĐD là cần thiết và có ý nghĩa thực tiễn.
Mục tiêu: Khảo sát kiến thức và kỹ năng chăm sóc NB có BTDD của ĐD tại BVUB
Phương pháp nghiên cứu: Nghiên cứu (NC) mô tả cắt ngang trên 139 ĐD tại 17 Khoa Lâm sàng, BVUB thành phố Hồ Chí Minh. Sử dụng thang đo SPSS 20.0 để khảo sát kiến thức và kỹ năng chăm sóc BTDD của ĐD. Phân tích tương quan Chi – square sử dụng để phân tích mối liên quan giữa yếu tố cá nhân, một số kiến thức và thực hành của ĐD
Kết quả: Tuổi trung bình của ĐD trong nghiên cứu là 35,3 ± 7,1 tuổi, thấp nhất 23 và cao nhất 56 tuổi, nữ chiếm ưu thế (89,9%) cao hơn nhiều so với các NC trước. Đa số ĐD có thâm niên công tác trên 10 năm (60,6%) và trình độ đại học chiếm 77%, cao hơn một số NC trong và ngoài nước. Về kinh nghiệm chăm sóc BTDD, nhóm có thời gian từ 1–<5 năm chiếm tỷ lệ cao nhất (47,5%). Có tới 92,1% ĐD đã từng tham dự tập huấn về BTDD, trong đó 56,1% chỉ tham dự 1 lần; nhu cầu được đào tạo thêm rất cao (98,6%).
Điểm trung bình kiến thức về BTDD ở mức đạt yêu cầu, tuy nhiên vẫn còn 4,3% thiếu kiến thức; đa số ĐD đạt mức kiến thức trung bình (94,2%) và chỉ 1,4% có kiến thức tốt. Về kỹ năng, cả kỹ thuật đặt và rút kim BTDD đều có tỷ lệ thực hành đúng >60%, trong đó nhiều bước đạt trên 95–100%, chỉ còn hạn chế ở khâu vệ sinh tay (74,8–80,6%). Tất cả ĐD đều đạt yêu cầu ở thang điểm xếp loại thực hành.
Phân tích yếu tố liên quan cho thấy: tuổi, trình độ chuyên môn, thâm niên công tác, thời gian chăm sóc NB có BTDD và việc tham dự tập huấn đều có mối liên quan có ý nghĩa thống kê với kiến thức và kỹ năng chăm sóc BTDD.
Kết luận: Nghiên cứu cho thấy đa số ĐD có kiến thức và kỹ năng chăm sóc BTDD đạt yêu cầu, tuy nhiên tỷ lệ kiến thức tốt còn thấp và vẫn tồn tại một số hạn chế. Các yếu tố như tuổi, trình độ chuyên môn, thâm niên công tác và tham dự tập huấn có ảnh hưởng rõ rệt đến kiến thức, kỹ năng chăm sóc. Vì vậy, cần tăng cường các chương trình đào tạo, tập huấn thường xuyên nhằm nâng cao chất lượng chăm sóc BTDD và giảm biến chứng cho người bệnh.
Abstract
Background: In recent years, subcutaneous venous access ports (SVAPs) have been increasingly utilized in cancer treatment due to their safety, low complication rates, and ability to improve patients’ quality of life. However, caring for SVAPs requires nurses to possess professional knowledge and practical skills. In reality, without proper training, nurses’ knowledge and practice levels reach only around 50%, which increases the risk of complications for patients. At the Oncology Hospital (BVUB), the number of patients receiving SVAPs continues to rise, yet the quality of care remains limited. Therefore, assessing nurses’ knowledge and skills in caring for patients with SVAPs is both necessary and practically meaningful.
Objective: To assess the knowledge and skills of nurses in caring for patients with subcutaneous venous access ports at the Oncology Hospital.
Research Methods: A cross-sectional descriptive study was conducted on 139 nurses across 17 clinical departments at Ho Chi Minh City Oncology Hospital. The SPSS 20.0 scale was used to assess nurses’ knowledge and skills in caring for patients with subcutaneous venous access ports (SVAPs). Chi-square correlation analysis was applied to examine the relationships between personal factors and selected aspects of nurses’ knowledge and practice.
Results: The average age of nurses in the study was 35.3 ± 7.1 years, ranging from 23 to 56 years old. Female nurses accounted for the majority (89.9%), a proportion significantly higher than reported in previous studies. Most nurses had over 10 years of work experience (60.6%), and 77% held a university degree—higher than figures found in several domestic and international studies. Regarding experience in caring for patients with subcutaneous venous access ports (SVAPs), the group with 1 to less than 5 years of experience represented the highest proportion (47.5%). A total of 92.1% of nurses had previously attended SVAP-related training, with 56.1% having participated only once. The demand for further training was notably high, at 98.6%.
The average knowledge score regarding SVAP care met the required level; however, 4.3% of nurses still lacked sufficient knowledge. Most nurses (94.2%) demonstrated moderate knowledge, and only 1.4% achieved a high level of knowledge. In terms of skills, both port needle insertion and removal techniques had correct practice rates above 60%, with many steps reaching 95–100%. The only notable limitation was in hand hygiene, with compliance rates between 74.8% and 80.6%. All nurses met the required standards on the practical skills rating scale.
Analysis of related factors revealed statistically significant associations between nurses’ age, professional qualifications, years of experience, duration of SVAP care, and participation in training sessions with their knowledge and skills in SVAP care.
Conclusion: The study indicates that most nurses possess adequate knowledge and skills in caring for patients with subcutaneous venous access ports (SVAPs). However, the proportion of nurses with high-level knowledge remains low, and certain limitations persist. Factors such as age, professional qualifications, years of experience, and participation in training sessions significantly influence nurses’ knowledge and skills in SVAP care. Therefore, it is essential to strengthen and regularly implement training programs to improve the quality of SVAP care and reduce complications for patients.
